Για τη σχέση του κάτω έγκλειστου σωφρονιστήρα με το γναθιαίο πόρο


Είναι γνωστό ότι η ακτινολογική, με πανοραμική ακτινογραφία, προεγχειρητική διερεύνηση της σχέσης των κάτω τρίτων γομφίων με το γναθιαίο πόρο, γίνεται με την αξιολόγηση ακτινολογικών σημείων που αφορούν τις ρίζες του σωφρονιστήρα και το γναθιαίο πόρο. Τέτοια σημεία, ενδεικτικά για τη στενή εγγύτητα ή την εμπλοκή του σωφρονιστήρα με το αγγειονευρώδες δεμάτιο του γναθιαίου πόρου είναι:

- Ακτινοδιαπερατές περιοχές ελαττωμένης πυκνότητας κατά μήκος και πέριξ των ριζών.
- Ρίζες με Απόκλιση, Στένωση ή Δισχιδές άκρο.
- Διακοπή της λευκής γραμμής του γναθιαίου πόρου.
- Απόκλιση ή Γωνίωση ή Στένωση του γναθιαίου πόρου.
- Υπερεπίθεση των ριζών με το γναθιαίο πόρο.

Σύμφωνα με τη βιβλιογραφία, από αυτά τα σημεία τα πιο σημαντικά, από προγνωστική άποψη για τυχόν μετεγχειρητικές ανεπιθύμητες αισθητηριακές επιπλοκές, είναι η ακτινοδιαπερατότητα των ριζών, η απόκλιση του γναθιαίου πόρου και η διακοπή της συνέχειας του γναθιαίου πόρου. Ο κλινικός μπορεί να ζητήσει σε αυτές τις περιπτώσεις περαιτέρω διαγνωστικό έλεγχο με υπολογιστική τομογραφία. Ωστόσο δεν έχει τεκμηριωθεί αν, σε τέτοιες περιπτώσεις αυξημένου κινδύνου, η επακριβής γνώση των τοπογραφικών σχέσεων στην περιοχή της εξαγωγής, μειώνει σημαντικά την πιθανότητα των επιπλοκών.

Στις εικόνες που ακολουθούν υπενθυμίζονται κάποιες από αυτές τις περιπτώσεις στις οποίες θα πρέπει να δίνεται ιδιαίτερη προεγχειρητική και χειρουργική προσοχή:
